Student Absences/Tardiness
- Please call or email the Nurse's office to report any absence. Messages can be left 24 hrs a day. Kindly send a note to your child's teacher upon their return to school.
- Please notify us if your child is going to be late
- If a student is absent more than 3 days due to illness, they will require an MD note to return to school
- Please have an emergency contact available to pick up your child during the school day if necessary and keep all numbers up to date. We want to be able to reach someone in the event that your child needs you. Emergency contacts must bring valid ID.

Parents and Guardians:
If your child is experiencing nausea, sore throat, stomach upset or ear pain, he/she may be ill. The following guidelines will help you make the decision to keep your child home.
PLEASE DO NOT SEND THEM TO SCHOOL IF THEY:
- Are awaiting strep or Covid results
- Are complaining of a sore throat and it appears red or inflamed
- Have had nausea or vomiting within the past 24 hours
- Have had a temperature of over 100 degrees in the past 24 hours
- Have an itchy, red eye with a discharge
- Have a suspicious rash
Per the NYS Department of Health, Covid-19 Isolation Precautions are still in effect:
If your child has tested positive for Covid-19, please notify the School Nurse or Main Office as soon as possible. There is still a 5 day isolation period for those with a positive test result.
- The New York State Department of Health is following the Centers for Disease Control and Prevention’s (CDC) isolation and precautions for people with COVID-19 guidance, which provides information to those who tested positive. If you were exposed to COVID-19, follow these CDC precautions.
Physical/Dental/Scoliosis Exams - DUE IN THE HEALTH OFFICE BY OCTOBER 1
- New York State Education Law requires a Physical and Dental for:
- New Students
- Kindergarten
- 1st Grade
- 3rd Grade
- 5th Grade
- The examination must be performed within 12 months prior to the first day of the school year
- A scoliosis exam is required for all 5th grade students

- Children who have not brought in their completed forms will be scheduled to be examined by the school physician
The district’s School Health Services program supports your student’s academic success by promoting health in the school setting. One way that we provide care for your student is by performing the health screenings as mandated by the State of New York.
During the school year, the following screenings are required and (if left undocumented by primary healthcare provider) will be performed at school:
Vision
- Distance and near acuity for all newly entering students and students in Kindergarten, Grades 1, 3 and 5.
- Color perception screening for all newly entering students.
Hearing
- Hearing screening for all newly entering students and students in Kindergarten, Grades 1, 3, and 5.
Scoliosis
- Scoliosis (spinal curvature) screening for all girls in grade 5.
Medication
- Medication can only be administered by the school nurse. A written order by a physician is required for all medication administered in school. This applies to both prescription and non-prescription medications
- Medications must be provided in their original container or pharmacy labeled container
- Medication orders MUST be renewed annually, this includes Tylenol, Advil, Benadryl, Epipens & Asthma Inhalers
PE/Recess Exclusion
- Children may be excused from PE or recess for one day with a parent note
- A note from your child's physician is required for longer exclusions or following injury, surgery or hospitalization
- A physician's note is required to clear a child for return to PE
Allergies & Asthma

If your child has any allergies to food, medication, or bee stings, please have your physician complete the Allergy Action Plan Allergy Action Plan 6-17 2.pdf This form (as well as an Epi-Pen for school) is required for any children who have been prescribed an Epi-Pen for their allergy
- If your child has asthma, please submit an Asthma Action Plan Asthma Action Plan 6-17 1.pdf and MD orders for your child's inhaler and spacer to the Nurse's Office.
